Add implementation of RTC_DCHECK_NOTREACHED equal to the RTC_NOTREACHED.
The new macros will replace the old one when old one's usage will be
removed. The idea of the renaming to provide a clear signal that this
is debug build only macros and will be stripped in the production build.

This is somewhat klugey, because it does the same checks at two
different layers in the stack, in different functions, which runs
the risk of making them out of sync. But it solves the immediate
problem.

Setting the rtp header extensions on the packet delivery thread
(currently worker, soon to be network), is now possible without
taking the hit of deleting and recreating the receive stream (and
rtp receiver and related state).

The extension is suggested to be used for signaling per target bitrate, resolution
and frame rate to a SFU to allow a SFU to know what video layers a client is currently targeting.
It is hoped to replace the current Target bitrate RTCP XR message currently used only for screen share.

This was just checked in all places were it was used, moving the check
into RtpRtcp reduces the boiler plate required at the call sites.
Also changing to always register and unregister extensions by URI to
synchronize the code in AudioSendStream with the code in RtpVideoSender.
This prepares for reducing the scope of ChannelSend.

This change adds the writing and parsing of the `abs-capture-time` RTP header extension defined at:
http://www.webrtc.org/experiments/rtp-hdrext/abs-capture-time
We are still missing the code to:
- Negotiate the header extension.
- Collect capture time for audio and video and have the info sent with the header extension.
- Receive the header extension and use its info.

When two-byte header extensions are enabled, kMaxId will change from 15
to 255. This CL is a refactor to remove the direct dependency between
memory allocation and kMaxId.
